Position Description

The Nurse Practitioner is a licensed practitioner who is responsible for managing health problems and coordinating health care for the patient in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status).

Job Duties:

Patient Care:
  • Facilitate new patient consultation and health history review.
  • Obtains and records a complete history for each visit to include medical, family, and social history.
  • Performs a complete physical examination and screenings as dictated by the history.
  • Orders/performs and analyzes appropriate diagnostic procedures.
  • Review and interpret test results.
  • Establishes a medical diagnosis and develops a comprehensive plan of care.
  • Prescribes a medical diagnosis and develops a comprehensive plan of care.
  • Prescribes appropriate medications as indicated in established NP protocols.
  • Educates the parents and/or the patient on the plan of care, to include nature of illnesses, rationale and descripion of plan, and possible side effects of treatment.
  • Provides comprehensive, age appropriate health education, anticipatory guidance, and counseling to parents and/or patients.
  • Refers to physicians and/or other health care providers and services as warranted by the patient’s condition and needs.
  • Assesses outcome of plan of care and modifies plan as needed.
  • Provides appropriate documentation of findings and plan of care in patient records.
  • Ensure patient records are kept up to date including test results, immunizations, condition changes, etc.
  • Provides both inpatient and outpatient documentation as needed by assigned physician such as H&Ps and Discharge Summaries.
  • Completes procedures as assigned by physician.
  • Deliver quality care while complying with company policies, safety regulations, and productivity goals.

Teacher:
  • Provides educational programs for patients, families and the community.
  • Possess age specific skills as needed for communication with parents and/or patients.
  • Precepts nursing students to the role of FNP.

Consultant:
  • Acts as a resource to staff, families, patients, the community, and other health care providers in the area of Family Health.
  • Maintains professional contact with other FNPs and health professionals.

Manager:
  • Maintains comprehensive plans of care for patients through collaboration and coordination of all involved elements of care.
  • Maintains professional relationships with other staff and physicians.

Researcher:
  • Reviews and incorporates current research into practice.
  • Actively participates in the research process.
  • Shares findings and expertise through presentation and publication.

Professional Growth and Evaluation:
  • Maintains current knowledge in new medical standards of care, legislation relating to health and professional issues, and health related conditions of the community through continuing education, community involvement and research.
  • Develops and maintains current protocols for practice.
  • Demonstrates professional accountability through periodic evaluations of performance through peer review, quality assurance programs, and self-evaluation.
  • Actively participates in professional nursing organizations.

Qualifications:
  • Graduate of an accredited school of nursing.
  • Graduate of an accredited Nurse Practitioner Program.
  • Current Advanced Practice Registered Nurse (APRN) Licensure in the State of Georgia.
  • Current Nurse Practitioner certification in specialty area.
  • Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS); Pediatric Advanced
  • Life Support (PALS) provider card if in Pediatric Clinic.
  • 5+ years nursing experience required.
  • Strong knowledge base of HIPAA and OSHA required.
  • Clinical experience required before completing nurse practitioner program.
  • Ability to operate as part of a team, alongside physician and other clinical staff.
  • Must be familiar with EHR medical charting system.
  • Compassion; medical aptitude; communication and problem-solving skills.
  • Strong computer skills.
  • Eligible for hospital privileges.
